Consent Forms

Kareo's new Consent Forms reduces the amount of time required for a patient to receive clinical care, eliminate the need for paper printing and storage, eliminate paper mistakes and mishandling, and reduce office staff's time administering paperwork so they can focus on keeping the practice profitable and full of patients.

As we continue to build out Kareo Patient Intake, we want to give the patients an opportunity to read and sign their consent forms in the comfort of their own home and on their own time before coming into a medical office.  Consent forms will save time and effort for both the patient and front desk staff through a seamless electronic experience. This process will also reduce paper storage needs, amount of paper purchased and printed, as well as paper misplacement and shredding costs.

Create Consent Form

To get started, a System Admin will need to create consent form(s).

  1. Hover over the User icon and click on Practice Settings. The Practice Settings menu opens.
  2. Click Patient Intake. The Patient Intake settings page opens.
  3. Click + New Consent Form. The New Form page opens.
  4. Enter an Internal Form Name, Form Title, and other pertinent information.
  5. Add, delete, or reorder the section to customize the layout.
  6. Click Preview to see what the patient's view or Save when finished.

Once completed, the front office staff can send consent form(s) through the Patient Intake workflow.


Send Consent Form

  1. Create a New Patient Appointment and click “Email Patient Intake”.
  2. Click the additional forms drop-down to select which consent form(s) to send the patient.
  3. Click Save Appointment. The Patient Intake is sent to the patient.

Once the patient intake is submitted, the form(s) will be saved to the patient's documents as a PDF for review.
