Clinical (EHR) Release Notes October 24, 2019

In October 2019, your Kareo Clinical account will be upgraded to the latest version.

2019 Quality Measure: Hospice Discharge

Kareo's new Hospice Discharge is integrated across 15 measures for the 2019 reporting year. If a patient is placed into Hospice providers can exclude those patients. Providers should check the Quality Measures Dashboard to ensure they are selecting the right measures.

To notate a Hospice discharge:

  1. Open the patient's facesheet.
  2. Click History in the left menu. The History page displays.
  3. Click the Hospitalizations / Procedures link. The Hospitalizations / Procedures page opens.
  4. Populate the Admission Date and Length of Stay.
    • The Admission Date can reflect either the day a patient entered the hospital or the date of an office visit.
    • Length of stay will depend on which you are documenting in Admission Date.
  5. If necessary, populate the related procedure and status.
  6. Click to select "Hospice".
  7. Select the Hospice Admission Date.
    • This date may be the same as the Admission Date if the patient is being placed in hospice following an outpatient visit.
    • This date may be the discharged date if the patient is being place in hospice following an inpatient visit.
  8. Select the Prior Event from the drop-down menu.
  9. Select where the patient was Discharge to from the drop-down menu.
  10. Click Save when finished.
