Navigate Patient Demographics

The Demographics page stores information that is used to identify a patient and is important for billing and claim submission. This section of the system includes data such as the patient's address, social security number, ethnicity, emergency contact, and primary provider. From this page you can view a list of the patient's past and upcoming appointments and activate the patient's access to the Patient Portal.

Access Patient Demographics

  1. Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box.
  2. Click to select the patient from the auto-populated results. The patient's record opens to the Face Sheet.
  3. Click Demographics in the left menu. The Demographics page opens.


Navigate Patient Demographics

Information in the Profile, Additional Info, and Contacts tabs can be edited. The Appointments tab is view only.

Click a tab at the top of the window to review the information.

  • Profile: Enter additional patient information, such as driver's license number, marital status, and languages.
  • Additional Info: Enter nationality, place of birth, preferred pharmacies, and primary provider.
  • Contacts: Enter responsible party and emergency contact. By default, the Responsible Party is set to "Self" and the Emergency Contact is set to "Responsible Party."
  • Appointments: View the patient's upcoming and past appointments.
  • Patient Portal: Activate the Kareo Patient Portal that allows a patient to view his or her problems, allergies, medications, lab results, and vital signs.
