View Patient Intake Email Sample

When users send intake forms to a patient, patients will receive an email with a link to the web forms.

The web form will be responsive-- allowing patients to fill out their intake forms on a computer or on their smartphones and tablets. Information in these forms map back to the Kareo Platform, which will allow for the practice to review and merge the information into a patient's record.

The link will expire when one of the following occurs:

  • At midnight past the appointment date
  • When a patient appointment has been moved to "Finished" tab (Checkout, No Show, Rescheduled, Cancelled status)
  • Patient's primary email is updated in the patient's account. If a patient has an active intake, a new intake email and new link will get sent to the patient. The old link will expire.


Patient Intake Email Sample

  1. Hover over the User icon and click on Practice Settings. The Practice Settings page opens.
  2. Click on Patient Intake.
  3. Click on Send sample patient intake to my email.

Note: The email template is not customizable.
