Review Screening Tools Intake

Screening Tools provides Mental Health practices the ability to send screening tools electronically for the patient to complete digitally. Screening tools currently available: GAD-7, PCL-5, and PHQ-9.

Invite the patient to complete the screening tool prior to their appointment. Once the patient submits the intake form, the front office can review and merge demographics and financial information into the patient's chart. Custom Forms, Medical History, and Screening Tool documents are saved to the patient's document section.

The screening tools can also be completed during the visit using Send Forms. The patient can complete the screening tools in the office using Kiosk or their mobile device.

During the patient appointment, the provider can review the patient's medical history and screening tools in a note.

Create Note

  1. On the Dashboard, click on the In Office tab. A list of checked in appointments displays.
  2. Click on the patient's appointment. The Appointment Card opens.
  3. Click Create Clinical Note to create a note with the default note type. The Note opens.
    • To select a different note type, click the drop-down arrow and select the desired note type.

Review Screening Tools

  1. Under the Documents section, click on the side panel icon next to screening tool name. The Screening Tool panel opens on the left side of the note.
  2. Review the submitted information and total score in the side panel.
  3. Document the results in the desired section of the note.
  4. Click X to close the Screening Tool panel and continue with the patient care.
