Review Medical History Intake

Medical Intake offers providers an easy to use and intuitive experience for capturing patient medical, surgical, family, social and hospitalization histories, in addition to patient's medications and allergies.

Invite the patient to complete the patient intake form prior to their appointment. Once the patient submits the intake form, the front office can review and merge demographics and financial information into the patient's chart. Custom Forms and Medical History documents are saved to the patient's document section.

During the patient appointment, a clinical assistant or provider can review the patient's medical history in a note.

Create Note

  1. On the Dashboard, click on the In Office tab. A list of checked in appointments displays.
  2. Click on the patient's appointment. The Appointment Card opens.
  3. Click on the drop-down arrow and click H&P. The H&P Note opens.

Note: It is recommended to use a H&P note to review Medical History. Other note types may require users to manually add history sections into the note for every patient. If desired, create a custom note to use a note specific to the practice's needs.

Review Medical History

  1. Under the Documents section, click on the side panel icon beside the Medical, Allergy, & History document. The Medical History panel opens on the left side of the note.
  2. Click on a section in the note to document the patient's history. The history section opens.

  1. Review the submitted information in the medical history panel and document the patient's history in the corresponding sections.
  2. Click Save when finished.
    • Document additional history sections as needed.
  3. Click X to close the Medical History panel and continue with the patient care.
