Check Status of Patient Intake
If a patient has been sent an email with a link to fill out patient intake forms, users will see the patient intake status in the expanded Agenda View and in the Edit Appointment view.
Not Sent to PatientIf intake forms have not been sent to patients, their patient intake status will be set to "Not sent to patient".

Not Submitted by PatientIf intake forms have been sent to patients, and patients have not yet completed and submitted them, their patient intake status will be set to "Not Submitted by Patient". To the right of this status, there will be two links labeled Print Forms and Resend.

Pending ReviewIf patients have submitted their intake forms, and they are awaiting review from the practice, their patient intake status will be set to "Pending Review". To the right of this status, there will be one link labeled Review.

MergedIf patients have submitted their intake forms and the practice has reviewed and merged them, their patient intake status will be set to "Merged". |

RejectedIf patients have submitted their intake forms and the practice has reviewed and rejected their forms, their patient intake status will be set to "Rejected". |
