Resend or Print Intake Form

Users can view patient intake forms pending submission from the patient, the appointment's email history,  resend or print an intake form on the Patient Intake view.

Once the email link has expired, the patient will be removed from the list.  A patient intake link will expire when an appointment is moved to the Finished tab or 24 hours after the appointment.

Navigate to Patient Intake

Hover over the Engage icon in the top menu and click Patient Intake. The Patient Intake window opens.

Review Pending Submission

  1. Click the Not Submitted by Patient tab. A list of the patient intake pending submission by the patient is displayed.
  2. Sort or search pending submission patient intake:
    • Sort by: Appointment Date or Date Last Sent
    • Provider: All or a specific provider
    • Search: Search by patient name
  3. To see the email history for the appointment, click in the empty area of the row. The following will display:
    • The current email on file.
    • The date and email the patient intake was sent to.

Resend or Print Patient Intake

  1. Click Resend to send another email to the patient with the link to the forms to the email on file. A green confirmation message displays.

  1. To print the patient intake form, click the drop-down arrow and click Print.
    1. A new window opens with the Patient Form PDF.
    2. Click the Print icon and follow the printer prompt.
