Changes can be made to a problem from within a note or on the Problems page in the patient's record.
Open a Note - Enter the first 2–3 letters of the patient's name in the top navigation bar search box.
- Click to select the patient from the auto-populated results. The patient's record opens to the Face Sheet.
- Click Notes in the left menu. The Notes page opens.
- Click Open on the right of an existing note or click Create Note to open a new one.

Edit a Problem - Click Assessment. The Assessment box opens.
- Click Include Problems. The Include Problems window opens to the Active problems in the patient's record.

- Click on the problem in the list that you would like to edit. The Edit Problem window opens.
- Click Inactive to move the problem to the Inactive list.
- Click to make the problem Type Acute or Chronic.
- Click to add a Start Date and/or End Date (Acute problems only) for the problem.
- Add notes about the problem in the Comments box. Make changes and click Save.
- To change an extension on an ICD-10 injury code with a seventh character, click Change Extension. Select the appropriate extension from the list displayed under the ICD-10 Problem field.
- Click Mark as Error to indicate that the problem was added in error. Learn more here.
