Create Amendment Note

Document the patient's request for an amendment to their record (e.g., offer corrections, additions, or updates).

Create Amendment Note

  1. Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box.
  2. Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
  3. Click Notes. The Notes page opens.
  4. Click the Create Note drop-down arrow and select Amendment.
    Note: Amendment is only available for selection when the note type is activated.

Document Amendment

  1. Click on the Source of Request section header to add a note using free-form text or a Text Shortcut. Then, click X to close the section.
  2. Click on the Request Details section header to add details of the request using free-form text or a Text Shortcut. Then, click X to close the section.
  3. Click on the Decision section header to place an "X" inside the brackets to indicate if the amendment request is Accepted, Rejected, or Under Review. If applicable, add a note using free-form text or a Text Shortcut. Then, click X to close the section.

Save Note

Save the note without a signature to continue editing. To finalize the note, complete the note.

  1. To allow additional revisions to the note, click Save & Close. The note is saved and the status is Open.
  2. To finalize the note, click Complete. The note is saved and the status is Signed.
