Historical vaccines that are added to Immunizations can be differentiated from non-historical vaccines in that they will appear in the list with a month and year only under the Administered column.
Navigate to Patient Chart / Immunizations - Enter the first 2–3 letters of the patient's name in the top search box.
- Click to select the patient from the auto-populated results. The patient's chart opens.
- Click Immunizations in the left menu. The Immunizations page opens.

Add a Historical Vaccine - Click + Vaccine at the top right. A details page opens.
- Click Add historical.
- Enter information for administering the vaccine and click Save when finished.
- Vaccine: Enter the first 3 letters of the vaccine. Select the vaccine from the auto-populated results.
- Type: Select the Type from the drop-down menu.
- Date: Select the month and year the vaccine was administered.
- Number in Series: Enter the number of the vaccine series, if known.
- Provider: Enter the provider who administered the vaccine, if known.
- Source of Information: Select the source from which the historical information was received.
- Comments: Enter any comments, if desired.
