Navigate Patient History

A patient's history is easily recorded by selecting from a list of conditions, creating custom items, or adding comments. A patient's history is accessed from the History tab of the patient's chart.

Access Patient History Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box. Click to select the patient from the auto-populated results. The patient's Facesheet page opens. Click History. The History page opens.

Navigate Patient History Details: View recorded history details. Last Updated: Date history details was recorded or updated. Add History: Click a section to add or update details: Past Medical History: Document the patient's blood type and prior medical condition(s). Past Surgical History: Document the patient's social history that may be medically relevant. Family History: Document the patient's family member health status and prior medical condition(s). Social History: Document the patient's social history that may be medically relevant. OB & Pregnancy History: When available, document the patient's obstetric and pregnancy history. Hospitalizations / Procedure: Document the patient's hospitalization or procedure. Long Term Care Facility: Document the patient's long-term care facility history. Hospice: Document the patient's hospice history. Implantable Devices: Document the patient's implantable devices history.
