Create a Custom Item

When documenting a patient's history, you can add a custom item that may be specific to your practice. Note that once entered, a custom item cannot be edited or removed, and it will appear for all users in your practice.

Navigate to Patient Chart / History

  1. Enter the first 2–3 letters of the patient's name in the top search box.
  2. Click to select the patient from the auto-populated results. The patient's chart / Face Sheet  opens.
  3. Click History in the left menu. The History page displays.
  4. Click on the History link that you want to add a custom item to. Custom items can be added to all History sections except OB & Pregnancy and Hospitalizations / Procedures.



Create a Custom Item

  1. Click +Custom Item. The Add Custom Item window opens.
  2. Type the Name of the custom item.
  3. Click Save. The item now appears under the Custom Items section.
