Add, Edit, or Delete Custom Item

When documenting a patient's history, you can add a custom item for Past Medical History, Past Surgical History, Family History, or Social History that may be specific to your practice. Custom items can be edited or deleted as necessary.

Access Patient History

  1. Enter the first 2–3 letters of the patient's name or date of birth (mm/dd/yyyy) in the top navigation bar search box.
  2. Click to select the patient from the auto-populated results. The patient's Facesheet page opens.
  3. Click History. The History page opens.
  4. Click Past Medical History, Past Surgical History, Family History, or Social History. The corresponding history page opens.

Add Custom Item

  1. Click + Custom Item. The Add Custom Item pop-up window opens.
  2. Enter the Name.
  3. Click Save when finished. The item now appears under the Custom Items section for all patients.

Edit Custom Item

  1. Click Edit Custom Items. The Manage Custom Items pop-up window opens.
  2. Click the edit icon. The Edit Custom Item pop-up window opens.
  3. Edit the Name.
  4. Click Save when finished.

Delete Custom Item

Custom items associated to a patient's history cannot be deleted.

  1. Click Edit Custom Items. The Manage Custom Items pop-up window opens.
  2. Click to select item(s) to delete.
  3. Click Delete. The Delete Confirmation pop-up window opens.
  4. Click Ok to confirm. The item is removed from the Custom Items section.
