Document Hospitalization or Hospice History

Document the patient's hospitalization, procedure, or hospice history.


Navigate to Patient Chart / History

  1. Enter the first 2–3 letters of the patient's name in the top search box.
  2. Click to select the patient from the auto-populated results. The patient's chart / Face Sheet  opens.
  3. Click History in the left menu. The History page displays.
  4. Click the Hospitalizations / Procedures link. The Hospitalizations / Procedures page opens.

Add Hospitalizations / Procedures

  1. Select the Admission Date.
  2. Enter the Length of stay.
  3. If the hospitalization or procedure is Related To the patient's problems, select the problem from the drop-down menu.
  4. To add a procedure, enter the first 3 numbers of the Procedure Code. Select the procedure from the list that displays.
  5. Select the Status of the procedure.
  6.  Add Comments, if necessary.
  7. Click +History Item to add another hospitalization or procedure.
  8. Click Save when finished.

Add Hospice

Hospice is an exclusion in 15 of the clinical Quality Measures.

  1. Select the Admission Date.
    • The Admission Date can reflect either the day a patient entered the hospital or the date of an office visit.
  2. Enter the Length of stay.
    • Length of stay will depend on which you are documenting in Admission Date.
  3. If necessary, populate the related procedure and status.
  4. Click to select "Hospice" to open additional fields.
  5. Select the Hospice Admission Date.
    • It may be the same date as the admission date if the patient is being placed in hospice following an outpatient visit.
    • Admission date may be the discharged date if the patient is being place in hospice following an inpatient visit.
  6. Select the Prior Event from the drop-down menu.
  7. Select where the patient was Discharge to from the drop-down menu.
  8. Add Comments, if necessary.
  9. Click Save when finished.
