Import Summary of Care

One of the long term objectives of the HITECH Act is to improve patient care coordination; this requires all healthcare providers to communicate with one another and share patient records. More comprehensive than the CCD/CCR file, information included in the Summary of Care is:

  • Basic Demographics
  • Problems
  • Social History (specifically smoking status)
  • Instructions
  • Allergies
  • Encounter Diagnosis
  • Care Plans
  • Functional/Cognitive Status
  • Procedures
  • Vital Signs
  • Medications
  • Medications Administered During Visit
  • Results
  • Immunizations

Navigate to Patient Chart / Immunizations

  1. Enter the first 2–3 letters of the patient's name in the top search box.
  2. Click to select the patient from the auto-populated results. The patient's chart opens.
  3. Click Immunizations in the left menu. The Immunizations page displays.

Import Summary of Care

  1. Click the Exchange drop-down menu and select Import Summary of Care. The Import Summary of Care window opens. 
  2. Click Browse to locate the file to import and click Open.
  3. In the Document Name field, type a name (for example, "Summary of Care from Dr. Sherwin") .
  4. Click Upload. The file uploads to the Documents section of the patient's chart.




  1. Click Documents on the left menu.
  1. Locate the file you saved and click Merge to the right of that line item. The Merge Wizard opens.
  2. Make your selections:
    1. Check all items you want to import. These will be indicated with the "Merge" icon.
    2. Uncheck any items you do not want to import. These will be indicated with the "Remove" icon.
  3. Click Save.
