More than 50% of all unique patients seen by the Eligible Professional (EP) during the EHR reporting period be provided timely access to view online, download, and transmit to a third party their health information subject to the EP's discretion to withhold certain information.
Measure 2- 2016
At least one patient seen by the EP during the EHR reporting period (or patient-authorized representative) views, downloads, or transmits to a third party his or her health information during the EHR reporting period.
Measure 2- 2017
More than 5% of the patients seen by the EP during the EHR reporting period (or patient-authorized representative) views, downloads, or transmits to a third party his or her health information during the EHR reporting period.
Exclusions for Both Measures
- Neither orders nor creates any of the information listed for inclusion as part of the measures
- Conducts 50% or more of his or her patient encounters in a county that does not have 50% or more of its housing units with 4Mbps broadband availability according to the latest information available from the FCC on the first day of the EHR reporting period.
For an EP to successfully attest for Objective 8, they need to invite more than 50% of patients to the patient portal. Patients who are activated can view their problems, allergies, medications, lab results and vital signs. They can also manage their accounts, message their providers, and authorize access for a guest to view their account. Once a patient has been activated, he or she will receive an invitation email with instructions to register.
Once patients are invited, at least one patient will need to either view, download or transmit their personal health records from the patient portal in order for the EP to receive create for second measure.
An EP must report the thresholds for this measure as generated by the Meaningful Use Functional Measures Report for the designated reporting period.
We recommend that a copy of your Meaningful Use Functional Measures Report is placed in your audit folder and retained for a minimum of 6 years after your attestation. Your audit folder will provide necessary verification if you are selected for a CMS Incentive Program audit.
