The MIPS eligible clinician is in active engagement with a public health agency or clinical data registry to submit electronic public health data in a meaningful way using CEHRT, except where prohibited, and in accordance with applicable law and practice.

To get started:
  • Contact your state registries:
    • Inform the registry of your intent to submit electronic data to them.
    • Obtain all the required information from them, including the contact name and phone number of the person who will be working on your request.
  • Create a support case with Kareo:
    • Provide the information received from your state registries (including name & phone number of their contact).
  • Complete the process with Kareo:
    • Your case with Kareo will be assigned to our specialist.
    • Our specialist will communicate with the contact at the registry and start/complete the interface setup.
    • Our specialist will contact you for testing and to complete the process.

Performance Score Weight:  10 points if “yes” to both measures, OR if one “yes” and one exclusion is claimed.  Then if no “yes” responses and two exclusions are claimed the points are redistributed to “Provide Patient Electronic Access to their Health Information”.
Definition of Active Engagement: The MIPS eligible clinician is in the process of moving towards sending "production data" to a public health agency or clinical data registry, or is sending production data to a public health agency (PHA) or clinical data registry (CDR).
  • Option 1 – Completed Registration to Submit Data: The MIPS eligible clinician registered to submit data with the PHA or, where applicable, the CDR to which the information is being submitted; registration was completed within 60 days after the start of the performance period; and the MIPS eligible clinician is awaiting an invitation from the PHA or CDR to begin testing and validation. This option allows MIPS eligible clinicians to meet the measure when the PHA or the CDR has limited resources to initiate the testing and validation process. MIPS eligible clinicians that have registered in previous years do not need to submit an additional registration to meet this requirement for each performance period.
  • Option 2 – Testing and Validation: The MIPS eligible clinician is in the process of testing and validation of the electronic submission of data. MIPS eligible clinicians must respond to requests from the PHA or, where applicable, the CDR within 30 days; failure to respond twice within a performance period would result in that MIPS eligible clinician not meeting the measure.
  • Option 3 – Production: The MIPS eligible clinician has completed testing and validation of the electronic submission and is electronically submitting production data to the PHA or CDR.

Immunization Registry Reporting Measure

The MIPS eligible clinician is in active engagement with a public health agency (PHA) to submit immunization data and receive immunization forecasts and histories from the public health immunization registry/immunization information system (IIS). All vaccines must be reported to the registry.  Registries are not only for reporting pediatric vaccines, but adult vaccines as well.

MIPS eligible clinician must open a support case with Kareo to initiate the registration process.

Exclusions: Any MIPS eligible clinician meeting one or more of the following criteria may be excluded from the Immunization Registry Reporting measure if the MIPS eligible clinician:
  • Does not administer any immunizations to any of the populations for which data is collected by its jurisdiction's immunization registry or immunization information system during the performance period.
  • Operates in a jurisdiction for which no immunization registry or immunization information system is capable of accepting the specific standards required to meet the CEHRT definition at the start of the performance period.
  • Operates in a jurisdiction where no immunization registry or immunization information system has declared readiness to receive immunization data as of 6 months prior to the start of the performance period.

Performance Score Weight: 10 points if “yes” to both Immunization Registry Reporting and Electronic Case Reporting measures, OR if one “yes” and one exclusion is claimed. Then, if no “yes” responses and two exclusions are claimed the points are redistributed to “Provide Patient Electronic Access to their Health Information”.  If two “no” responses, then zero (0) for both Provide Patient Electronic Access to Health Information and Promoting Interoperability Performance Category.
Additional Information: The measures under the Public Health and Clinical Data Exchange objective are reported using “yes or no” responses. The MIPS eligible clinician will receive the full 10 points for reporting two “yes” responses, or for submitting a “yes” for one measure and claiming an exclusion for another. If there are no “yes” responses and two exclusions are claimed, the 10 points will be redistributed to the Provide Patients Electronic Access to Their Health Information measure.

Reporting on more than one of the three optional measures for this objective will not result in more than 5 bonus points.

Electronic Case Reporting Measure

The MIPS eligible clinician is in active engagement with a public health agency (PHA) to submit electronic case reporting.

Note: Kareo providers can claim exclusion for calendar year 2022 as Kareo works to implement electronic case reporting.

Exclusions: Any MIPS eligible clinician meeting one or more of the following criteria may be excluded from the Electronic Case Reporting measure if the MIPS eligible clinician:
  • Does not treat or diagnose any reportable disease for which data is collected by their jurisdiction’s reportable diseases system during the performance period. OR
  • Operates in a jurisdiction for which no public health agency is capable of receiving electronic case reporting data in the specific standards required to meet the CEHRT definition at the start of the performance period. OR
  • Operates in a jurisdiction where no public health agency has declared readiness to receive electronic case reporting data as of 6 months prior to the start of the performance period. OR
  • Uses CEHRT that is not certified to the electronic case reporting certification criterion at (for the CY 2022 performance period/CY 2024 MIPS payment year only) 170.315(f)(5) prior to the start of the performance period they select in CY 2022.

Bonus: Syndromic Surveillance Reporting Measure

The MIPS eligible clinician is in the process of moving towards sending "production data" to a public health agency or clinical data registry or is sending production data to a public health agency (PHA) or clinical data registry (CDR).
