Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-up Plan

Percentage of patients aged 18 years and older with a BMI documented during the current encounter or during the previous twelve months AND with a BMI outside of normal parameters, a follow-up plan is documented during the encounter or during the previous twelve months of the current encounter.

Normal Parameters: Age 18 years and older BMI => 18.5 and <25 kg/m2

Denominator: All patients aged 18 years and older on the date of the encounter with at least one eligible encounter during the measurement period.
  • Patient’s age is calculated based on the patient’s Date of Birth, which is entered when you created the patient’s account.
Denominator Exclusions:
  • If "Pregnancy" is documented in the Problem List, with a Start Date, these patients will be excluded from the denominator.
  • If Patient Refused BMI Measurement select Patient refuses weight and/or height from Vitals, then click Save.  These patients will be excluded from the denominator.
  • Exclude patients whose hospice care overlaps the measurement period.
Numerator: Patients with a documented BMI during the encounter or during the previous twelve months, AND when the BMI is outside of normal parameters, a follow-up plan is documented during the encounter or during the previous twelve months of the current encounter.
  • BMI is calculated based on Height and Weight entered in Vitals
  • "Abnormal BMI Follow-up" is documented on the Care Checklist.
Office Visit Documentation:

Once all steps have been completed, the measure will be added to the Quality Report.

Guidance: The most recent BMI will be used to determine if the performance.
