Service Location : Entity's Postal/Zip Code

 

 

 

 

This rejection indicates that an invalid service location zip code is being sent on the claim. Please verify that the 9-digit zip code submitted on the claim is valid, some payers will not accept "0000" as the last four digits of the code.  

 

 

Follow the instructions below to verify the sevice location zip code:

  1. Click Settings > Service Location.
  2. Look for and double-click on the service location that needs correcting.
  3. Enter an address in the Address field with a valid 9 digit zip code.
  4. Use the USPS Zip Code Look Up tool to validate the 9 digit zip code.
  5. Click Save.

Then, rebill and resubmit all affected claims.
