Rejection Details

Kareo Validation Error

This rejection indicates the state abbreviation used in the address for one of the following is invalid/incorrect:

  • The Practice
  • The Insurance Plan
  • The Insured (if other than the patient)

You can verify which of the above has the invalid abbreviation based on the content of the Rejection Message.

Claims billed without an address for any of the the above will reject at the clearinghouse; therefore Kareo has held the claim until the error has been corrected. You must correct the error and resubmit the claims in order for them to be delivered to the payer.

Note: Since claims rejected within Kareo are not forwarded to the clearinghouse you will be unable to generate timely filing for that submission until validation errors have been corrected and the claim resubmitted.

Resolution

Resolution steps will vary depending on the cause:

If the practice's address needs to be verified, follow the instructions below:

  1. Click Settings > Practice Information. The Practice Information window opens.
  2. Enter the address on file under the Contact Information into the USPS ZIP Code Look Up tool.
  3. Enter the correct information into the Address field under the Contact Information.
  4. Click Save.

Then, rebill and resubmit all affected claims.

If the payer's address needs to be verified, follow the instructions below:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double-click on the encounter that needs correcting. The Edit Claim window opens.
  3. Double-click on the Case. The Edit Case window opens.
  4. Double-click on the appropriate Insurance Policy. The Insurance Policy window opens.
  5. Double-click on the Insurance Name. The Edit Insurance Plan window opens.
  6. Enter the address on file into the USPS ZIP Code Look Up tool.
  7. Enter the correct information into the Address field.
  8. Click Save until you get to the Edit Case screen.
  9. If there are multiple policies in the case, follow steps 4-8 for each policy.
  10. Click Save all the way out (multiple saves may be required).

Then, rebill and resubmit all affected claims.

If the insured's address needs to be verified, follow the instructions below:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double-click on the encounter that needs correcting. The Edit Claim window opens.
  3. Double-click on the Case. The Edit Case window opens.
  4. Double-click on the appropriate Insurance Policy with an insured. The Insurance Policy window opens.
  5. Enter the insured's address on file into the USPS Zip Code Look Up tool.
  6. Enter the correct information into the Address field.
  7. Click Save all the way out (multiple saves may be required).

Then, rebill and resubmit all affected claims.
