Payer Responsibility Sequence Number Code cannot occur more than once within a claim

Rejection Details

This rejection indicates that there are multiple or duplicate payers listed on the claim and their sequence numbers are not listed or unique.

Sequence numbers correlates with the patient's coordination of benefits. 

1 = Primary
2 = Secondary
3 = Tertiary  

Resolution

Follow the instructions below to deactivate an insurance policy:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double-click on the encounter that needs correcting. The Edit Claim window opens.
  3. Double-click on the Case. The Edit Case window opens.
  4. Double-click on the Insurance Policy that needs to be deactivated. The Insurance Policy window opens.
  5. Click to clear "Active" checkbox.
  6. Click Save all the way out (multiple saves may be required).

Then, rebill and resubmit all affected claims.
