Rejection Message

Discharge Date (DTP-01=096) was not expected because this claim is not for Inpatient Services.

Rejection Details

This rejection indicates a hospitalization discharge end date was included in an outpatient claim.

Discharge dates are only expected for Inpatient Services.

Resolution

There are two places where the date may have been entered. Follow the appropriate steps below to remove the date.

Follow the instructions below to remove the discharge end date from the encounter:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double-click on the encounter that needs correcting. The Edit Claim window opens.
  3. Double-click on the Encounter number. The Edit Encounter window opens.
  4. Click the arrows next to Hospitalization Dates to expand.
  5. Highlight the End Date and click Delete on your keyboard.
  6. Click Save & Rebill.

Then, resubmit all affected claims.

Follow the instructions below to remove the discharge date from the patient case:

  1. Click Encounters > Track Claim Status. The Find Claim window opens.
  2. Look for and double-click on the encounter that needs correcting. The Edit Claim window opens.
  3. Double-click on the Case. The Edit Case window opens.
  4. Click on the Condition tab.
  5. Right-click on the Hospitalization Related to Condition and click Remove.
  6. Select Hospitalization Related to Condition in the Date Type drop-down.
  7. Enter the Start Date.
  8. Click Add.
  9. Click Save all the way out (multiple saves may be required).

Then, rebill and resubmit all affected claims.
